Job description

Gade Surgery has a fantastic opportunity for Advanced Nurse Practitioner to join our team on a full-time basis, for 37.5 hours per week, to utilise your expert clinical knowledge and skills. 
 
You will have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients, providing an opportunity for patients to receive timely care and negate unnecessarily delay in receiving treatment and without the need to refer to a GP, developing close, long-term relationships with your patients and working in partnership with them to achieve optimum health. You will be autonomous in making decisions based on assessment, diagnosis and interpretation of test results and independently prescribe appropriate medication, evaluate or refer to other specialists if necessary. 
 
The main responsibilities of the role include:
  • Developing, implementing and embedding health promotion and wellbeing programmes.
  • Managing patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
  • Implementing and evaluating individual specialised treatment plans for chronic disease patients]Identifying, managing and supporting patients at risk of developing long-term conditions, preventing adverse effects to the patient’s health.
  • Providing advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
  • Providing wound care (ulcer/Doppler etc.).
  • Providing specialist clinics such as ear syringing, contraception etc.
  • Undertaking the collection of pathological specimens.
  • Providing travel medicine services.
  • Requesting pathology services as necessary.
  • Processing and interpreting pathology and other test results as required.
  • Providing chronic disease clinics, delivering patient care as necessary, referring patients to secondary or specialist care as required.
  • Maintaining accurate clinical records in conjunction with extant legislation.
  • Ensuring read/SNOMED CT codes are used effectively.
  • Maintaining chronic disease registers.
  • Developing, implementing and embedding well woman/man/travel.
  • Be responsible for the implementation of an effective immunisation programme.
  • Chaperoning patients where necessary.
  • Supporting the team in dealing with clinical emergencies.
  • Recognising, assessing and referring patients presenting with mental health need.
  • Implementing vaccination programmes for adults and children.
  • Be an extended and supplementary prescriber, adhering to extant guidance.

Request

All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Master’s degree required for qualification post December 2020.
  • Meets the standards for registered ANP working at advanced level.
  • Minor illness qualification
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.  The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
