Anp/Acp - Care Home Team Wigan

Hi,

We are looking to recruit an ACP/ANP to support PCNs in Wigan as part of their Care Home Team.

The service is designed to develop and embed positive, proactive care for people in Care Home settings, including nursing and residential home settings for adults with social, physical, and mental health needs and people with learning difficulties. In a typical day, you would visit 2 Care Homes,

The team includes an ACP, 2 paramedics, 2 pharmacy technicians, care co-ordinator and a lead GP.

You must be able to assess and treat varied clinical cases, have advanced skills to assess the patient with a differentiated or undifferentiated clinical diagnosis.

Must also be a prescriber.

Full MSc /Digital Badging required

Start Date: As soon as possible

System- EMIS / SystemOne (training and refreshers can be provided)

Rate - £45ph

Days- Looking for 2-3 days a week, 3 month contract initially, ongoing until March 2025 with the right candidate

Hours/Times - Between 8:00 am - 6:00pm - flexible on number of hours

Days - Flexible on the days - Mon, Tues, Thurs, Fri
