The post is for a qualified Nurse working 35 hours per week, based at Heartlands and Solihull Hospitals, but you will have to have flexibility to cover other hospital sites (Good Hope Hospital) as required.

You should have significant relevant post registration experience including within cardiology with evidence of continued professional development. You should have experience and/or a keen interest and understanding in the rehabilitation of cardiac patients and of those with chronic conditions. You will have excellent communication skills, be organised, a good team player and have time management skills to work under pressure alongside other members of the team. You will be expected to work autonomously and be able to make decisions.

This post will suit a dynamic and enthusiastic candidate who has a positive outlook and is able to motivate patients to make lifestyle changes.

Main duties, tasks & skills required

The Cardiac Rehabilitation team provides a vital role in the recovery of patients who have suffered Acute Coronary Syndrome (STEMI & NSTEMI) and to those who are post coronary revascularisation (PCI and CABG surgery).

We provide a comprehensive service and as part of your role you will provide education to patients and their families' pre discharge, developing an individualised pharmacological and non-pharmacological plan for their Cardiac Rehab and communicate this effectively with primary care, to ensure a seamless service is offered to patients. You will also be required to support patients in the out-patient Cardiac Rehabilitation programme providing both health education and exercise supervision and guidance both remotely and face-to-face alongside experienced Clinical Exercise Physiologists. This includes assessing patients' suitability and risk stratification for core rehab (phase 3) and refer onto the most appropriate programme depending on patients risk and choice in conjunction with the Multidisciplinary team.
