Hays Healthcare are actively recruiting Band 6Community Mental Health Nurse for our older people service in Bury (Manchester). This role offers full-time and part-time hours.
Your new roleand responsibilities
- Undertake care coordination and care management.
- Assesses needs and identifies problems relevant to the care of people referred to.
- Devises a plan of care in partnership with the person and carers.
- Has continuing responsibility for the provision of skilled interventions to people with Mental Health problems in the community. Works within a multidisciplinary team as a member of a CMHTOP service.
- Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
- Implements the planned programme of care to ensure a high standard is achieved.
- Advises and supports the person and carers to promote health and well-being and to prevent illness.
What you'll need to succeed
- Up-to-date Professional Registration (to be maintained), RMN
- Evidence of continuing professional development in the field of Mental Health
- Experience with older adults with Mental Health problems and lone working
- Ability to apply therapeutic knowledge to clinical practice
- Experience in Mental health and risk assessments
- Community Experience
