Care Co-Ordinator

Location: Tameside/Ashton-under-Lyne

This is a band 6 position available for an experienced care co-ordinator who is looking to work in the community

Start date: ASAP-3 months

Shift pattern: Monday-Friday, 9:00-17:00

Hours: Full time, 37.5hrs/week

Job description:

• The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community.
• Develop clear relapse recognition / prevention plans and share with MDT, service users and carers where appropriate
• Implementation of a plan of care for the service user in collaboration with the multi-disciplinary team, referring agent, family/carer and service user, where appropriate
