Job description

DCV Technologies is looking for a Care Coordinator in Hemel Hempstead to work closely and as part of the care team you will ensure that our clientsreceive excellent standards of care and support through effective allocation of care givers. You will direct, mentor and supervise the team to ensure excellent standards are maintained and that care is delivered in a timely way in line with client’s individual needs and wishes.

MAIN RESPONSIBILITIES

  • Allocating care givers with the right skills and experience to deliver the best quality care and support to each client.
  • Organise Care givers rota's to minimise changes to the service and travel between client homes.
  • Responding efficiently to day-to-day changes in the care and support packages.
  • Implementation and monitoring of all relevant aspects of the Electronic Call Monitoring System.
  • To ensure that all clients are correctly allocated on colleague and call times reflect the care package. To co-ordinate with deputy manager and ensure new care packages and care plans are completed within agreed timescales.
  • Establish strong working relationships with all key stakeholders, including commissioning teams, Safeguarding Teams, social workers, procurement, District Nurses, OT's and PT's.
  • Start-up, administration and review of new support packages, Client Reviews, Care giver supervision & support, Care giver training (including recommendations for specialist courses), Support with care giver recruitment.
  • Spot checks on Care giver competency and on quality-of-care provision in partnership with the Care Manager
  • To bid for new care packages and work closely with the management to ensure that new care packages have the required care givers cover.
  • To achieve targets, KPI's and objectives set by the Care Manager.
  • To be conversant with the Care Standards Act 2014 and legislation governing the service and other regulations concerning the provision of domiciliary care.
  • To be familiar with the Company's obligation under the Health and Safety at Work Act 1974 and other Health and Safety Regulations detailed in the company's Health & Safety Policies.
  • Carrying out visits to clients’ homes to carry out initial Client Assessments and continuous monitoring.
  • Liaison with other agencies (eg Social Services) regarding service provision.
  • Any other duties as reasonably required by the Manager. The Care Coordinator will be required to assist with administration and on-call (out-of-hours telephone)
  • Ability to work alone and within a team

PERSON SPECIFICATION

  • Experience of supervisory management in the care provision sector.
  • Experience of client care/handling complaints/problem solving.
  • Ability to work as part of a team.
  • Excellent communication skills, both written and verbal.
  • Very good interpersonal skills.
  • Ability to manage difficult situations calmly and effectively.
  • Ability to lead and motivate care givers.
  • Knowledge of Care Quality Standards of Care,
  • Knowledge of, and commitment to, person centred approaches to care.
  • Knowledge of IT systems - Microsoft office: Word, Excel, Outlook etc.
  • Knowledge and experience in rostering
  • Social Care Qualification – Level 3 minimum
  • Mandatory training
  • Ability to work alone and within a team

ADDITIONAL INFORMATION

Full driving licence & car owner. Flexible and adaptable to work across multiple sites and on various projects. Subject to DBS check and references.

Hold business class insurance class 1
