Our client, a leading care provider, is seeking a compassionate Care Coordinator to join their team near Newton Abbot. Renowned for their dedication to enabling individuals to stay comfortably in their homes, they're distinguished by their commitment to personalised care.
Operating from the heart of Newton Abbot, our client requires a Care Coordinator to plan carers' assignments, ensuring exceptional support for customers in their homes.
This will be a full time permanent role which is split coordinating and also carrying out assessments in customers homes.
Responsibilities as a Care Coordinator:
- Assign care professionals to clients, ensuring personalised and timely care in alignment with individual preferences detailed in care plans. Uphold top-quality care, emphasising safety and comfort at clients' residences. A composed, supportive demeanour, coupled with excellent organisational and communication skills, is vital for this pivotal role.
- Produce required reports as directed by the Registered Manager/Directors. Use IT systems to allocate suitable care professionals, considering clients' needs. Collaborate with supervisors to match professionals with clients, factoring in preferences, logistics, and evolving care requirements. Effectively prioritise during emergencies, promoting client well-being and fulfilment.
While prior experience with Care Coordinator systems is beneficial, a natural inclination towards compassion, care, and patience is key. They seek a team that genuinely cares for each client as if they were family. Rest assured, comprehensive training will be provided to ensure readiness for responsibilities.
Essential Requirements:
- Strong organisational, IT and planning skills
- Excellent communication abilities
- Right to work in the UK
- Proficiency in English
- Able to work every other weekend
- Have access to own transport as you will be required to travel to Exeter at least once a week
