Are you a passionate, dedicated Care Manager Assistant looking for your next role? 4Recruitment Services are recruiting a Care Manager Assistant to join the Education Care and Health Services Team based in Bromley.
Umbrella Rate: £18.66-£23.96 per hour
Duration: 3 month initially
18 hours per week, 8.45am to 5pm
Information about the role:
- Undertake assessments of Adults and Carers.
- To provide practical support and information for Adults and Carers.
- Create individual support plans by arranging and monitoring the delivery of services/payments to Adults and Carers.
- Monitor and review support plans and the effectiveness of services provided for Adults and Carers
- To work with team colleagues and other professional staff to ensure that service user’s needs are met.
- Request minor adaptations for the Service Users home and arrange for adaptive equipment whilst demonstrating how to use correctly
- To ensure all relevant parties involved with Adults and Carers are notified of changes and/or amendments to support plans.
- To maintain Adult and Carer records in accordance with the departmental policy and practice.
- To deal with complaints.
- Working alongside other organisations and agencies.
- Assist registered staff colleagues, when required, on complex care pathways e.g., safeguarding investigations, and cases where there is a high level of risk.
- Participate in the operation of duty and respond accordingly to need
- Assist in the induction of new staff
- Be responsible for making appropriate safeguarding alerts, in conjunction with professional staff
Requirements:
- Able to demonstrate experience of having worked within Health and Social Care directly with Adults and Carers
- Ability to undertake an assessment for residential care
- Experience of both short-term and long-term assessments and case management
- Minimum standard of English GCSE Grade A -C or equivalent
- UK Driving License, access to a vehicle and business insurance.
- DBS registered to the update service.
