Carry out nursing therapeutic work with complex patients with a learning disability and mental health disorder Work collaboratively with other members of the multi-disciplinary team (MDT) to ensure that treatment plans are in place and clear lines of accountability are established. This work will be undertaken in the context of multi professional and interagency working In conjunction with the MDT, oversee the day to day delivery of treatment strategies to patients ensuring that all multi professional treatment recommendations are implemented and evaluated in a timely manner Undertake comprehensive assessment and treatment planning and provides direct nursing interventions, advice and evaluation to the Secure Service ensuring that the care planning process does not delay the patient progress through the treatment pathway Ensure that care plans are appropriate to the needs of the patients, person centred and reviewed in a timely manner that reflects the changing needs of the patient. Ensure the utilisation of evidence-based assessments, formulations and nursing therapeutic interventions, which appropriately meet the needs of patients on an individual and group basis Ensure that they Care and Treatment Review (CTR) process is followed and that quality documentation is provided in line with national guidance. Ensure that the patient pathway follows the discharge checklist proces Maintain therapeutic relationships with patients within clear boundaries incorporating the principles of relational security (the knowledge and understanding staff have of a patient and of the environment and the translation of that information into appropriate responses and care) throughout the service Actively participate in the on-going development of the service through multi-professional and interagency networking. Actively participate in the Quality Network Forensic Mental Health Services (QNFMHS) peer review process, supporting the development and completion of relevant action plan Provides appropriate specialised nursing support and advice to patients families and carer Contribute to developing and maintaining a safe and therapeutic environment for patients and staff based on models of care as identified by the MDT and national standard Provide clinical reports that reflect the specialised gender specific knowledg Motivate and encourage nursing staff and other professional colleagues in reviewing their practice to support innovation and change and optimise clinical effectiveness Recognise own limits seeking appropriate specialist support and supervision Promote a positive and open culture both within the clinical areas and across the servic Maintain the physical, procedural and relational security of patients, colleagues and self in line with current policy and practic Contribute to maintaining security, observing local policies and procedures in relation to risk Ensure the safe use and care of security equipment, e.g. Pinpoint Alarms, two-way radios, mobile phones etc Provide cover in the absence of the ward manager ensuring that all operational functions of the Secure Service are maintained during the period of absence Take the lead in ensuring that appropriate clinical supervision is provided to all nursing staff working within the Secure Servic Identify and where appropriate, meet the training needs of colleagues Design, develop and implement appropriate training and development programmes for staff across the Brooklands service based on specialist therapeutic knowledge and expertis Be involved in the recruitment and selection of all grades of staff within Secure Services
