Job Description:

Are you a skilled social worker with a passion for making a difference in the lives of disabled children and their families? We are seeking a motivated individual to join the Disabled Children's Team in Stockton on Tees.

Responsibilities:

• Conducting thorough assessments of the needs and circumstances of disabled children and their families.
• Developing and implementing individualized care plans that address the specific needs and goals of each child.
• Providing support and advocacy for disabled children and their families, ensuring they have access to necessary services and resources.
• Collaborating with healthcare professionals, educational staff, and other relevant agencies..
• Maintaining accurate and detailed records of assessments, care plans, and interventions in line with legal and professional standards.

Qualifications:

• Qualified Social Worker registered with Social Work England.
• Experience working with disabled children and their families, preferably within a social care setting.
• Strong assessment, planning, and intervention skills.
• Excellent communication and interpersonal skills, with the ability to build trusting relationships with children and families.
• Knowledge of relevant legislation, policies, and procedures related to disability and child welfare.
