DiaDem (Diagnosing Advanced Dementia Mandate) is a dementia diagnosis application aimed for use in care home settings to be used with people living with advanced dementia within a care home who do not have a formal diagnosis. This is a relatively new service designed to enhance Demetia Diagnosis Rates in care homes in the East of Berkshire.
Work base: You will be based in Maidenhead, but will be working in care homes across the East of Berkshire.
Parking: There is free staff parking at the main site.
Working pattern + Service operating hours
The role will cover Monday – Friday 9am – 5pm with some flexi-time considered. There will be some flexibility for remote working ie report writing, virtual supervision etc
- DiaDem is a tol to support the diagnosis of dementia for people living with advanced dementia in a care home setting. The post holder will be responsible for working with care home staff and GP’s to identify those residents who do not have a recorded diagnosis of dementia and where there are concerns around advanced memory difficulties
- The post holder will be responsible for liaising with the resident, care home staff and family members to complete the DiaDem diagnosis assessment, under the supervision of the consultant psychologist.
- To work closely with administrative support to liaise with GP’s, families and care homes to book assessments of appropriate residents.
- The post holder will be responsible for ensuring those diagnosed with dementia receive appropriate support, including dementia friendly care, orientation and communication strategies for staff.
- The post holder, under the supervision of the consultant psychiatrist will highlight those residents who would be suitable for review of antipsychotic medication
