About the role
These two posts will lead a new team that aims to improve the outcomes of people who are frequently using NHS urgent and emergency care services.
This team is hosted by Cambridgeshire County Council on behalf of the wider Integrated Care System, and will trial a more proactive and personalised approach.

What will you be doing?
You will be responsible for:
- Developing, leading and managing a high performing team of practitioners
- Implementing a new approach to support people who are using lots of unplanned services but no one partner has been able to solve their problems
- Building strong relationships across the voluntary, public, and private sectors
- Share and using learning to make improvements to existing policies and processes
About you
You will have a track record of working in partnership within a complex network of organisations to design and deliver a service safely and effectively. You will have experience of leading a team, and a demonstrable understanding of health inequalities and a commitment to the delivery of personalised care.
You will be comfortable and capable at building trusted and collaborative relationships.
You will be passionate about championing improvements to how we support people and leading improvements to services, processes and policies as needed
