Job Summary:
We are seeking a compassionate and dedicated Community Care Worker to join our team. As a Community Care Worker, you will provide support and assistance to individuals in need, helping them to live independently and improve their quality of life. Your role will involve mentoring and supervising clients, ensuring their well-being and promoting their self-sufficiency.
Duties:
- Provide direct care and support to individuals in the community
- Assist clients with daily activities, including personal hygiene, meal preparation, and medication management
- Develop and implement care plans based on individual needs and goals
- Monitor client progress and make adjustments to care plans as necessary
- Provide emotional support and companionship to clients
- Assist with transportation to appointments or community activities
- Collaborate with other healthcare professionals, social workers, and family members to ensure coordinated care
- Maintain accurate documentation of client interactions and progress
Requirements:
- High school diploma or equivalent; additional education or certification in a related field is a plus
- Proven experience in mentoring or supervising individuals
- Strong interpersonal skills and the ability to build rapport with clients
- Compassionate and patient demeanor
- Excellent communication skills, both verbal and written
- Ability to work independently and as part of a team
- Valid driver's license with a clean driving record
