Job Description:
Are you a compassionate and skilled Social Worker looking for a rewarding opportunity in healthcare? We are seeking experienced Hospital Social Workers to join our team in North Tyneside.
In this role, you will play a crucial part in providing support and assistance to patients and their families, ensuring they receive the care and resources they need for a smooth transition during their hospital stay and beyond.
Responsibilities:
Conducting comprehensive assessments of patients' social and emotional needs, including factors such as housing, finances, and support networks.Collaborating with healthcare professionals, patients, and their families to develop and implement personalized discharge plans.Providing advocacy and support to patients and their families, helping them navigate healthcare systems and access relevant services and resources.Liaising with community agencies and service providers to coordinate ongoing support and follow–up care for patients post–discharge.Maintaining accurate and up–to–date records of assessments, interventions, and discharge plans.Qualifications:
Qualified Social Worker registered with Social Work England.Experience working in a hospital or healthcare setting, preferably in a social work capacity.Strong assessment and communication skills, with the ability to engage effectively with patients and their families.Ability to work collaboratively as part of a multidisciplinary team.Empathy, resilience, and a commitment to providing high–quality care and support to patients and their families.Benefits:
