Develop Mental Health Medicines Management (MHMM) services in the community sector of the Trust. Lead on the development of Mental Health Medicines Optimisation programme. Co-ordinate care from secondary into primary care, working closely alongside local Community Mental Health teams (CMHT) and charities. Include the development of specialised medicines management support for Mental Health patients and include medication review, monitoring and adherence support. 5 years post registration experience including 3 years in hospital of which 2 years must be at band 7 or above

MPharm or equivalent and eligible to register with PSNI

Experience in mental health clinical pharmacy.

Qualified as a Pharmacist Independent Prescriber.

Post graduate qualification in a mental health/psychiatric related subject area or be enrolled in such a course.
