Are you passionate and committed to supporting Older People with functional and organic Mental Health problems?
We are looking for motivated and enthusiastic Registered Mental Health nurse with specialist interest and experience in caring for Older People with Mental Health problems to join our CMHT for Older People. The post is full time.
As a well-established CMHT with motivated, proactive and supportive team members, we care for service users with complex needs, including severe and enduring Mental Health needs, dementia and cognitive impairment, requiring us to provide skilled assessment and interventions through the integrated Health and Social Care model of care.
We are currently undergoing an exciting time of transformation, working in partnership with our service users, their carers, Local Authority, voluntary organisations and other stakeholders in a drive to continue our goal of providing inclusive and high quality care while developing our service beyond the traditional scope of health and social care.
The Team promotes opportunities for professional development through regular supervision, access to trainings, including managerial and leadership and quality improvement (QI) projects.

To be an active member of the multi-disciplinary providing assessment, care and support to Older People presenting with functional and organic Mental Health problems and other co-morbid health care needs within the London Borough of Newham.
Actively engage people with functional and Cognitive impairment and their carers in all stages of care provision, ensuring that their views are acknowledged and taken into account.
Assess and present individual service user cases in MDT meetings to review their care needs.
To laisse with the GPs, Local Authority and other statutory organisations and voluntary agencies to promote partnership working.
To act as a care coordinator for a caseload of service users, ensuring that you develop, implement and review the individualised care provided.
To conduct carers assessment and take action where required, including offering of support and referral to other services.
To act as a duty officer in line with the Team Single Point of Entry (SPE) duty Rota, processing referrals and responding to emergencies as they arise.
Take part in the Transformation Process of the Team in line with the NHS Long Term Plan.
Ensure that service users receive a culturally sensitive care.
Participate in QI Projects, audits and surveys relevant to the Service
To proactively engage supervision and appraisal and provide supervision to junior colleagues.
