Job description

An exciting opportunity has arisen for a Band 6 Community Mental Health Nurse to join the Newham Community Recovery - South. The service aims to provide care to the service users of Newham, working in partnership with Community Integrated Mental Health Service (CIMHS), the Primary Care Network (PCN) structure in Newham and third sector provision. The service offer a flexible and accessible approach to working with service users to achieve their goals towards recovery. The role will involve working collaboratively with CRT colleagues as well as a MDT consisting of Psychiatrists, Nurses, OT’s, Community Connectors, Peer Support Workers, Clinical Associate Psychologist and many more. Working with a large MDT provides a platform for peer learning and professional development.
Key Responsibilities includes:
Providing assessment and recovery focused care planning in conjunction with the multi- disciplinary team.
You also participate in the setting up and running of service user and carer groups, which promote confidence building, meaningful occupation and recovery.
You will provide a service which promotes recovery, identifies people’s needs and strengths, is service user centered and recovery focused, and promotes safety and positive risk taking.
Ensuring the service provided is of the best quality and of a standard of professional practice and performance to meet the needs of Newham’s diverse community. Provide a service which adheres to policies and procedures.
The successful candidate will manage of a personal caseload as agreed with the Operational Team Lead and within the set referral criteria. Act as a care coordinator in line with Care Programmed Approach, CPA Policies. Provide assessment and recovery focused care planning in conjunction with the multi- disciplinary team. Participate in the setting up and running of service user and carer groups, which promote confidence building, meaningful occupation and recovery. Provide a service which promotes recovery, identifies people’s needs and strengths, is service user centred and recovery focused, and promotes safety and positive risk taking. Candidate will also ensure the service provided is of the best quality and of a standard of professional practice and performance to meet the needs of Newham’s diverse community. The candidate will provide a service which adheres to policies and procedures of the trust. You will provide a service which is of the highest standard in terms of fair access, equality of opportunity and anti-discriminatory practice. You will also act a placement supervisor or assessor to students on placement.

PERSON SPECIFICATION
Education/ Qualification/ Training
RMN Qualification, BSc or Masters/Registration NMC (PIN)
Evidence of regular CPD
Experience
Well-developed and suitable experience of working as a qualified RMN Suitable experience of working in a Community Mental Health setting. Experience of completing full psycho-social assessments and writing reports.
Experience of working in a multi-cultural, inner city area.
Develop team links and ensure effective working practice with all related Health and Social Services, voluntary, education and employment agencies.
Knowledge and Skills
The current legislation relating to Mental Health provision.
The needs and rights of service users.
Psychiatric problems and the social,
psychological and political dimensions of mental ill-health.
Current models of working with people with serious Mental Health.
Experience with problems including psychosocial and recovery approaches.
Current research and how this can inform practice.
The major current issues in relevant health and social care.
To engage with people with serious Mental Health problems in a holistic manner.
To assess their needs, design and deliver appropriate interventions and evaluate the effectiveness of the process.
To communicate effectively in English, both orally and in writing
To work effectively as part of a Team and facilitate productive working relationships.
To manage time, be self-motivated and to work both independently and as part of a team as required.
To use supervision positively and effectively.
To liaise effectively with other agencies, statutory and non-statutory
To teach and supervise junior staff and students
Word-processing/Computer skills
Other requirements
An understanding of the principles and practice of Equal Opportunities and a commitment to work within that framework
Ability to meet the health requirements of the East London.
