SalaryTo provide skilled nursing care working as part of the Primary Health Care Team to patients in a variety of community settings between the hours of 08:00 and 20.30, including weekends. To actively monitor, maintain and develop the service provided. To take responsibility for managing the caseload and the team, with the support of the Team Leader. To deliver a proactive case managed approach for patients with long term care needs. To actively participate in service development activities.
