Requirements
An exciting opportunity has arisen within Hamilton Community Mental Health Team for a qualified Social Worker to join our team. We would welcome applications from enthusiastic, confident individuals who are looking for an opportunity to work in a multidisciplinary Mental Health Team. We are looking for a Social Worker with post qualifying experience, preferably within a Health and Social Care Partnership and or with experience of working with adults affected by mental illness.
If successful you will work as an effective member of a multi disciplinary team, alongside Mental Health officers, psychiatrists, community psychiatric nurses, support workers and occupational therapists. Your role will involve completing community care assessments under the Self Directed Support (Scotland) Act 2013, incorporating careful consideration of risk; arranging, monitoring and reviewing corresponding care packages. You will be responding to safeguarding concerns and carrying out relevant statutory duties, for example in relation to the Adult Support and Protection (Scotland) Act (2007).
You will work alongside Mental Health Officers and allied health professionals to carry out assessments to support hospital discharges for service users who might be subject to legislation such as the Mental Health (Care and Treatment) (Scotland) Act (2003); and the Adults with Incapacity (Scotland) Act (2000). You should have excellent written and verbal communication skills in order to complete all informal, formal reports and case notes which need to be of a high standard.
