Job description

Here at Cheshire West and Chester Council, we are committed to providing excellent Adult Social Care services to residents. We are one team, putting what matters to people first and are investing in a number of roles, across our service areas to grow our dynamic, solution focused team.
So, wherever you are in your life, there is something to suit you in Adult Social Care, whether that be meaningful work, feeling valued in your role, career progression, pay progression or a great work life balance, this is where you will find it. If you want to be a part of our welcoming, values driven service and are passionate about being part of a team that can make a real difference for local people, please take a look at the roles we have available.
This is a unique and exciting opportunity for any social worker wanting to join this specialist team who work closely with the Adults with Learning Disability social work teams, Transitions team, CHC team, Mental Health teams and colleagues in Commissioning. Office bases are in Ellesmere Port, Chester and Winsford and the expectation is that staff work in the office 1-2 times a week and at home or out on visits the remainder of the week.
The Complex Care and Review team is a small but extremely effective team consisting of a Practice Manager, 3 x Social Workers, 2 x Social Care Assessors, 1 x Occupational Therapist and 1 x OTA. This advert is for 3 further SCAs and a further OT will shortly be advertised due to expand the team. This is an experienced team of highly motivated, hardworking, and skilled professionals. We are looking for candidates who demonstrate these skills and have a passion to drive forward positive outcomes for individuals regardless of their level of need or stage of life they are in. Candidates will be required to work collaboratively and creative with colleagues, and to have an in-depth knowledge of the Care Act 2014 and a sound knowledge of current legislation, policy and procedures within adult services.
The team is unique in that it completes reviews taking a ‘whole service/provider’ approach. In doing so they work closely with providers and commissioners to improve the standards of current services. This includes setting clear outcome focused care plans, taking an innovative approach regarding effective use of current services, and scrutinising services to ensure commissioned support is required and being used appropriately in keeping with a person-centred and strength-based approach. Their remit also includes project work, working closely with operational teams and commissioners in the future forecasting of service-user’s needs within CW&C, identifying gaps in services and steering new bespoke commissioning opportunities.
Experience with learning disabilities and/or mental health is preferred and experience in supported living, day services, residential care would be beneficial. Full training and support will be given.
Our values are important to all of us, we live and breathe them, and they shape how we work together to create positive environments where both colleagues and people who use our services can be inspired to reach their potential. If you share our values and commitment, then you’re our kind of person.
You will have:
NVQ Level 3 in Health and Social care or equivalent (Desirable).
Be required to undertake an enhanced DBS plus Adults Barred List (Essential).
Experience of working with vulnerable people and their carers.
Ability to communicate effectively with customers, carers and families.
