We are currently seeking a dynamic Social Care Coordinator to join our Mental HealthSocial Care Team, working closely with our Hoarding Specialist Social Worker in our friendly and supportive Multidisciplinary Team.
In the role of Social Care Coordinator at Reading Borough Council, you will play a crucial role within a friendly and cooperative team. You will collaborate with other Social Care Coordinators, Social Workers, Occupational Therapists and professionals from the voluntary and community sector.
The Mental HealthSocial Care Team aims to actively promote wellbeing and independence, working with people with complex Mental Health difficulties in partnership with health colleagues. Supporting people to retain or regain their skills and confidence by using existing and new support networks, Technology Enabled Care (TEC), equipment and short-term interventions.

About the Role:
This is a new and unique role to support the health and wellbeing of people who Hoard. In the role of a Social Care Coordinator, your will work closely with people who hoard, using a collaborative and strengths-based approach to implement interventions to reduce risks and support positive outcomes.
You will hold a caseload of people who Hoard and will be expected to work independently with support and supervision from the Hoarding Specialist Social Worker. Due the nature of the work you will need to have a good understanding of risk management; managing yourself in difficult environments; flexible and able to analyse varied information or situations to produce solutions.
You will work within the legal framework provided by the statute, guidance, policies, and procedures.
Role requirements.
- Experience working with people.
- Experience working within the Health, Adult Social Care or voluntary sector – (not essential)
