Social Care Practitioner

We are a close and supportive team of Social Workers, Social Care Practitioners, Care Advisors, Senior Practitioner and Service Manager. Our team members have manageable caseloads, and we recognise the importance of robust reflective supervision.
You will be encouraged to share your ideas, exhibit best practice and actively work towards improving the work that we do.
We work with adults over 18 and above with a variety of presenting needs, which provides opportunity for stimulating and interesting caseload.
Your role
We are looking for an experienced Social Care Practitioner to join our team. As a Social Care Practitioner in the team, you will work with residents of Bath & North East Somerset who may be in need of Social Care support.
You will primarily be undertaking assessments under the Care Act 2014 and Mental Capacity Act 2005. This involves direct work with individuals, families and professionals, report writing, support planning and providing short term support.
About You
You must have a recognised health and Social Care qualification e.g. NVQ 4 or equivalent, or nursing qualification, or allied health professional qualification.
You will have experience of working with adults in need of support and have a working knowledge of effective models of strength based personalised assessments, knowledge of current research and a working knowledge of current legislation.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
Ability to manage a caseload and have effective communications skills.
You will be based primarily in Peasedown St John, Bath but you may need to travel as part of the role.
