Responsibilities:
- Conduct holistic assessments of adults experiencing Mental Health difficulties, considering their social, emotional, and practical needs.
- Develop and implement person-centered care plans in collaboration with service users, promoting recovery, resilience, and independence.
- Provide therapeutic interventions and support services to help adults manage symptoms, improve coping strategies, and enhance their quality of life.
- Work collaboratively with Mental Health professionals, including psychiatrists, psychologists, and community Mental Health teams, to ensure coordinated and integrated care.
- Advocate for the rights and preferences of service users, ensuring they have access to appropriate support, resources, and opportunities for inclusion and participation.
- Monitor and review care plans regularly, adapting interventions as needed to reflect changes in the service user's circumstances and goals.
- Facilitate access to community resources, housing, employment, education, and other support services to promote holistic recovery and social inclusion.
Requirements:
- Qualified Social Worker with current registration with Social Work England.
- Proven experience working with adults experiencing Mental Health issues, preferably within a statutory Mental Health setting.
- Sound knowledge of Mental Health legislation, policies, and best practices related to adult Mental Health care.
- Strong assessment, communication, and interpersonal skills, with the ability to build rapport and trust with individuals from diverse backgrounds.
- Ability to work collaboratively within multidisciplinary teams and across different agencies to achieve positive outcomes for service users.
- Compassion, empathy, and a non-judgmental approach to supporting individuals experiencing Mental Health challenges.
- Commitment to continuous professional development and adherence to ethical standards and professional boundaries.
