To support the inpatient medical wards as the named physician responsible for patient care. The post holder will be supported by a senior Consultant who will provide informal advice routinely and will also provide prospective cover during planned periods of leave. To lead a team of MDT professionals: Two Specialty doctors based at each ward will provide day-to-day medical cover. The Specialist will be responsible for routine ward rounds and management plans for patients under their care. To provide medical leadership support for the health and social care services that make up the MDT. To work closely with other Consultants and Managers to deliver effective and appropriate care services to residents of the borough. The post holder will perform a key role in discharge planning and co-ordinating approaches with relatives and other agencies To liaise with patients, relatives/carers and members of the multi-disciplinary team to ensure patients receive a high-quality service and all kept informed of progress as appropriate To be responsible for contemporaneous record-keeping, identifying progress and plans in patients case notes and on the Clinical Portal. To lead ward rounds, facilitating Falls multidisciplinary meetings and ward rounds. To complete accurate and timely discharge summaries, progress reports and ensure General Practitioners are informed of any impending discharges, changes of medication and death. To actively participate in multi-disciplinary audit, clinical effectiveness/health gain initiatives, Clinical Governance, local clinical audit activities and developing research projects, as appropriate To support the pan Gwent Clinical Model of the Gwent Frailty Programme; work patterns may change and the place of duties modified To review results in a timely manner and refer to other specialities as appropriate To undertake teaching and training as part of job role To support research studies.
