The role is varied and will include: Identifying the health needs and barriers to accessing appropriate services and support. To undertake health assessments on Children and Young People (CYP) in accordance with agreed procedures. To make appropriate referrals to other services or follow up investigations to ensure this vulnerable group does not lose their place on waiting lists or disrupt existing arrangements. To assess health needs holistically and to develop a health plan that relates to the individual CYP considering their physical, emotional, behavioural, social and developmental needs. To support care givers to implement the health plan. Review health care plans in the light of the individual C/YP circumstances. To ensure health plans are implemented both in and out of borough. To ensure that health services for looked after children are delivered in accordance with service specifications. To ensure that services to Looked after Children are developed in accordance with national guidance and policies. To ensure that services for Looked after Children and Care Experienced/ Leavers are provided and developed in line with current evidence-based practice. To effectively work with residential settings both in and out of borough. To promote independent living for young people exiting care; aiding as the role necessitates. To work with accommodation providers for looked after children and Care experienced adults providing advice / support as appropriate. Provide support, advice, referral and advocacy to the leaving care population. Undertake tasks appropriate to band of post to support the Safeguarding Team. Provide support and guidance to more junior members of the Safeguarding Team inclusive of the Administrators The post holder will also be required to deliver bespoke Children In Care training. Additionally:Providing advice (direct and indirect) to colleagues on the assessment, treatment and clinical services for all aspects of Safeguarding & Children in Care Providing appropriate professional advice on concerns including within the wider context of child health and social care and contributing to the Operational/strategic and ongoing planning of Childrens Services within Walsall Healthcare NHS Trust. Encouraging and facilitating a culture that involves and empowers staff, stimulating innovation and active participation in shaping servicedevelopments for the delivery of patient care. Ensuring the Trusts aims and objectives are maintained, engaging when required in groups, forums, meetings as delegated by the Named Nursefor Children in Care ; deputising as necessary in operational/strategic meetings. Providing cross-cover and support across the Safeguarding Service in line with the post-holders professional accountability and expertise. Ensure that oversight of the Administrative Team exists. Ensuring that Walsall Healthcare NHS Trust has a training strategy in line with the Intercollegiate Document and local expectations. Ensuring there are robust and up to date local policies and guidelines are available throughout Walsall Healthcare NHS Trust.
