Job description

Role summary To autonomously provide specialist nursing care within their own competence and work with and support the Advanced Specialist Nurse, Physiotherapist and Occupational therapist, to advise and facilitate high quality, individualised care to patients and their carers in collaboration with the multi/interdisciplinary team (MDT) for patients with Parkinsons Disease, Tremors, Complex Tremors and Dystonias who are referred to the service. This role will have a strong focus on assessing and treating patients with movement disorder who have been referred and then undergo DBS therapy. Trouble shooting, forward thinking and implementation of treatment plans and changes for the problems that arise, will be a prominent skill required throughout the patients pathway. As a key member of the MDT, facilitating care and treatment tailored to each patient is necessary to achieve an improved quality of life for patients undergoing DBS for Movement Disorders. Previous experience of working with movement disorder patients and demonstrating a keen interest is desired, together with a forward thinking MDT working approach and excellent communication skills to excel in this role. Key responsibilities Establish and manage a clinical caseload as appropriate to the service with support from the Advanced Nurse/Occupational Therapist/Physiotherapist To undertake all aspects of clinical duties as an autonomous practitioner. To influence and manage the delivery of Nursing/Occupational/Physiotherapy care input to patients with long-term neurological conditions within the Functional Neurosurgery Neuromodulation Service. To ensure specialist clinical assessment, intervention and signposting based on evidenced based practice. To work collaboratively with other clinicians, including medical and nursing staff to influence service development and ensure a high quality of care is delivered. Following assessment, where appropriate to deliver specialist intervention or refer to colleagues within the wider MDT who have the appropriate skill to deliver the intervention To provide clinical education and training to other health care professionals. Support research, innovation and professional development for self and others. Clinical Responsibilities 1/ To be flexible in both approach to the clinical area and work load layout, acting as a professional role model, providing direct and indirect clinical care, teaching to patient's families and other health professionals. 2/ Competently assess, plan, implement and evaluate patients and their treatment throughout the pathway and their long term condition, with support of the Advanced Nurse/Occupational therapist / Physiotherapist. 3/ Actively seek evidence based practice which will improve the quality of care and meet the needs of a group of patients. A holistic bio-psychosocial approach to caring should be planned that will include the appropriate level of physiological; pathological; spiritual, socio-economical and cultural aspects which influence the care of clients within your specific specialty. 4/ Take an active role in intraoperative assessments and programming. Including assessment of movement disorder, programming of DBS and trouble shooting. 5/ Facilitate short and long-term follow up of patients through provision of Nurse-led clinics, telephone triage, day case admissions; facilitating consultant-led clinics and admissions as required under the guidance of the team. 6/ Programme and support maintenance of Deep Brain Stimulation devices 7/ Have and activity use a non-medical prescribes qualification or be willing to work towards a non-medical prescribing course, and advising patients as inpatients and out patients for their full treatment plan- with support from the team. 8/ Ensure that accurate and comprehensive records are kept and confidentiality is maintained. 9/ Take responsibility for all actions and practices in accordance with the NMC code of conduct, Trust and local policies. 10/ Support the advanced Nurse/Occupational/Physiotherapist in undertaking the development of professional practice policy/guidelines 11/ Manage episodes of care as own/joint shared caseload, discharging patients as per protocol. 12/ Maintain and update own clinical expertise to ensure the delivery of therapeutic/nursing care is at highest possible level to meet patient care. 13/ To act as patient advocate in line with the NHS plan and to actively apply in depth knowledge of holistic/socio-economic and cultural aspects of the care to clients within your specific specialty, acting as a role model and resource for patients, relatives and staff. 14/ To assist on the implementation of the Trusts framework for Clinical Governance within own clinical specialty. Clinical Governance 1. To comply with health and safety policies of the Trust including adhering to Lone Working Procedures and undertaking all Trust mandatory training at the required intervals 2. To be involved in the development of departmental procedures and guidelines on areas of clinical relevance. 3. To contribute to and take appropriate action in seeking to fulfill the action plans of the WCFT in compliance with clinical governance and national guidelines 4. To ensure that any equipment, furniture or building in need of repair is reported to the correct authority 5. To comply with the Chartered Society of Physiotherapy standards and rules of professional conduct or to comply with the Nursing and Midwifery Council, The Code, Professional standards of practice and behavior for nurses, midwives and nursing associates. 6. To be involved in clinical audit through developing and actively participating in audits within the neurosurgery, neurology and neuromodulation service. 7. To promote best clinical practice for the Nurse/Occupational/Physiotherapy service ensuring a high standard of patient care is provided. 8. To be involved in the development of care pathways / standards of care for people with complex Movement Disorders, primarily Parkinsons Disease under the care of the centre, reflecting national and European standards 9. To comply with the Data Protection Act and Caldicott recommendations Research and Development 1. To participate in research studies of relevance to the DBS service. 2. To analyse relevant research in regard to clinical practice. 3. To contribute to the quality of the DBS teams service Training and Development To actively seek out clinical supervision and take responsibility for ensuring continuation of the process. To be involved in the organisation of seminars, study days and meetings led by the Movement Disorder/DBS service. To attend relevant courses and meetings both for personal development and the development of the DBS service through evidence based practice and to disseminate the information obtained at such course/meetings as required. To contribute to the training programme of Doctors and other Health Professionals. To support teaching and provide specialist advice to carers and other Health Professionals to ensure excellent provision of patient care. . Managerial & Leadership 1. Maintain awareness of current evidence of best practice to develop care and introduce changes with colleagues and act as a role model in evidence based practice. 2. Monitor the pathway of care followed by all patients throughout their treatment, referring to other healthcare professionals and services as appropriate. 3. Facilitate the links with other healthcare professionals including medical staff, in the hospital and community care setting to maximise continuity of care for patients. 4. Network at local, regional, national and international level. 5. Contribute to the development and effective implementation of Trust policies.
