Core responsibilities and duties include:-
Frontline delivery of an ‘all age person-centred’ holistic service in a locality, to help people lead healthy and active lifestyles, which allows them to remain living independently in their homes for longer.
Ensuring individuals have choice and control in how their support is provided.
Customer facing delivery of early intervention and prevention solutions to reduce escalation to costly services.
To tackle low level crime, anti-social behaviour, hate crime and provide effective support to victims and vulnerable residents utilising enforcement tools and powers alongside targeted interventions.
Using robust partnership analysis, data and effective intelligence reporting for hotspot action planning.
The communities’ team are the local coordinators for the Government Programme - Stronger Families here in Doncaster. In addition to this, officers proactively identify and manage less intensive cases as part of the Stronger Families Programme as well as supporting lead practitioners at higher thresholds by completing interventions to support significant and sustained change.
Directly contribute to the delivery of the Well Being agenda to support people to access local services, drive innovation around the community offer and enable residents to live independently in their homes longer and reduce the need for costly services.
Key link with the emerging work around Complex Lives / Troubled individuals and vulnerability where impact is seen in the wider area.
Work with a range of partners, residents, and services to promote and influence on early intervention and prevention, Community Led Support and Community Capacity building. To develop an understanding of the needs of local people which will enable communities to do more for themselves and develop their own solutions.
Progress casework for anti-social behaviour, stronger families and wellbeing and support vulnerable and repeat victims of anti-social behaviour, hate crime and crime using an holistic outcomes based approach.
Support the delivery of community projects using various forms of funding Work with local ward members to support them in their community leadership role.
