You will be part of the PfA Team, an integrated health and social care team within the Integrated Learning Disabilities Service. Your role will involve supporting young individuals with learning disabilities, aged 18 to 25 or until their EHC Plan ends, in their successful transition into adulthood. You will assess their suitability for support before they turn 18, ensuring a seamless transition that begins before children's services support concludes.
