The post-holder will support the service leads in delivering service changes that will contribute to the overall delivery of the NEL Health and care partnerships strategic plan and improve outcomes, experience and overall value for money from commissioned services. The post holder will have responsibility for delivering significant elements of the Care and Independence team plan and/ or the corporate plan as agreed. The work will include taking a lead on highly complex and contentious projects or work streams which will draw on specific technical or legal areas of knowledge or practice, or areas of national/ local policy to interpret and implement. The post holder will be required to work across a full range of service lead areas covering health and social care e.g. hospital discharge processes, long term care, support to care homes, re-ablement and support at home. The post holder will also be required to take the lead on existing commissioned services ensuring they continue to deliver to the specification requirements and are meeting the needs of the local population. This may include the need to re-tender services which have reached their contract term.
