1. Job Summary Work alongside partners within the NEL Health and Care Partnership to lead specific service transformation programmes, with a focus on frailty and integrated neighbourhood teams Work with colleagues to support the delivery of a range of other programmes and projects across the North East Lincolnshire Place, as required Systematically engage with, and influence a range of providers and experts by experience to support the delivery of sustainable service change and integrated care, through effective stakeholder management Work with providers and experts by experience to identify and minimise barriers to service development and/or gaps in service Support the delivery of local health and care outcomes including positive service user experience and improved service quality; ensuring that these improvements reflect the local and national strategic aims Be responsible for leading the delivery of specific service improvement areas/projects as defined. This will involve being the lead and subject matter expert for dedicated areas of work within agreed programmes Provide a robust programme management approach to support the delivery of the key impact areas within the NEL Health and Care Partnership, working alongside the HCP programme team members Working with complex provider arrangements, lead partners from across the HCP to develop the Integrated Neighbourhoods Team to deliver proactive frailty across NEL To consider the impact of any change the projects will have on the system and support wide engagement across the system to secure full ownership and buy-in. To ensure the flexibility of the projects, if required to meet conflicting/changing requirements Day to day line management of project team members as required.
