Church Langley Medical Practice has an opportunity for a care co-ordinator to join our team, to support our patients within the practice to navigate care provision.
The main duties of the role include:
- Supporting the practice manager in coordinating patient feedback, inclusive of all compliments and complaints, and summarising learning to be shared practice wide.
- Supporting digital initiatives.
- Assisting in prevalence of patients care records.
About the Candidate
If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
The ideal applicant will have the following skills, experience and qualifications:
- Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
- Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification.
- Strong listening and communication skills.
- Familiar with coding records.
- Motivated by helping people, with care and empathy.
- Able to deliver person-centred support in a non-judgmental way.

We are a medium sized practice with a high percentage of younger patients, a lot with children. Based in Church Langley we have free parking on our door step in the Tesco’s car park. Church Langley is a work in progress, we are striving to update technology available and modernise the way we work, a positive attitude and fresh ideas are very much welcome within the team.
