Introduction to the Role:
The CROAL Team are looking for a Care Coordinator to oversee and provide assessment, planning, of care to a group of service users within Croydon. Within this post, you will work effectively within a multi-disciplinary team and across other agencies, to provide a high quality of care to individuals with complex bio psychosocial needs.

The South London & Maudsley NHS Trust provides a full range of mental health services to people of Southwark, Lewisham, Lambeth and Croydon as well as substance misuse services for people in
Bexley, Greenwich and Bromley. The Trust provides specialist services to people from across the UK.
It has approximately 4,800 staff and a budget of £300 million.
Assessment & Liaison Team provides a comprehensive mental health service to adults of working
age (18-64 years) within the London Borough of Croydon and is one of the four London Boroughs
receiving Mental Health Services from the South London and Maudsley NHS Foundation Trust.
Responsibilities
· To assess needs, plan, implement and monitor care in collaboration with the service user, carers, and other members of the multidisciplinary team.
· To implement care that meets the mental, physical, spiritual, psychological and social needs of the service user, and that is sensitive to age, culture, race, gender, ethnicity, social class, sexuality and disability.
· To demonstrate an understanding of the distress caused by, and social and personal consequences of mental illness for service users and their careers.
· To formulate plans of care with service users which actively promote activities of health promotion and recovery.
· To actively promote relationships with colleagues that support open communication and collaborative working, taking personal responsibility for one's contribution to the process of MDT working.
To be successful in applying for this role you will need:
· HCPC registration
· Registered Nurse (Mental Health) or other professional registration e.g. Social Work and Occupational Therapy.
· Experience of working in Home Treatment Teams or Psychiatric Liaison or equivalent environment.
In addition to experience, all Qualified roles will be required to provide evidence of:
- Relevant Qualifications
- Registration with the relevant Registration Body if applicable.
