Case Manager | City Health Care Partnership CIC

The opportunity has arisen to recruit to a band 6 Case Manager for the Bridlington/Driffield Nursing and Conditions Team based at Bridlington. The post is a permanent position working hours 30 hours per week. The ideal candidate must be a Registered Nurse with a Specialist Practice Qualification in District Nursing or have experience working as part of the Community Nursing Team. This is an exciting opportunity for a reliable, flexible and enthusiastic experienced nurse who is committed to providing high standards of care and has proven leadership skills.
Applicants should have experience in acting as role model, supervisor and assessor, supporting continuous professional development and promoting high standards of care. Applications are welcome from experienced registered community nurses who are able to demonstrate excellent communication and organisational skills as well as the ability to case manage an identified group of patients within a community nursing setting.
