Position: Hospital Discharge Social Worker
Pay Rate: £35-£38 per hour
Contract: Temporary, Full-time
Responsibilities:
- Coordinate safe and timely discharges from hospital
- Assess the needs of patients and their families
- Develop and implement care plans
- Liaise with health professionals and community services
- Provide support and advice to patients and their families
Requirements:
- Degree in Social Work
- Registered with the Scottish Social Services Council (SSSC)
- Experience in Hospital Discharge planning
- Knowledge of relevant legislation and policies
- Excellent communication and interpersonal skills
