Job description

The starting salary for this role is GBP38,746 per annum based on a 36-hour working week. This opportunity is offered as an 18-month fixed-term contract/ secondment opportunity.
We operate a policy of hybrid working, which means you will have the option to spend part of your week working from home whilst ensuring a regular presence is maintained in the office.

We have a great opportunity for a DSR & Keyworking Business Lead to join and work across two Health and Social Care Teams in our growing but friendly team supporting Children and Young People (0-25 years) across Surrey.

The two teams are responsible for supporting children and young people experiencing crisis or who are inpatients either in Tier 4 mental health provision, 52-week residential care, or at risk of admission. In partnership with their families and the health, education and social care system, you will break down barriers to ensure children, young people, and young adults with a learning disability and/or autism access the care and support they need.
About the role
This newly created role is joint between Surrey County Council and the NHS, bringing together the Learning Disabilities & Autism Team (NHS) and the newly established Keyworking Team (SCC).
You will be responsible for maintaining and developing the Dynamic Support Register (DSR), as well as looking at moving towards a digital platform training of professionals across education, health and social care teams. The role also encompasses co-production with various stakeholders, including people with Lived Experience, Carers and statutory organisations.
This will be a challenging but rewarding role as developing the DSR will be a vital part of ensuring these teams can deliver support in a timely and efficient way. You will need to ensure that all duties are carried out to a high standard in liaison with the clinicians and stakeholders of the DSR, alongside the organisation, and attend multi-agency tracking meetings.
As the lead of the DSR you will ensure that the referral route is effective, the DSR is functional for the teams, and that it meets national guidelines. You will create assurance reports and data dashboards, whilst using your interpersonal skills to work closely with partners to raise awareness of the DSR and provide training where necessary.
As part of your role you will liaise with keyworkers, mental health workers, local authorities, service users and their families, specialist learning disability and/or Autism providers and local commissioners.

We're looking for someone who will feel confident both working autonomously and delivering training in a group face-to-face environment to a range of health and social care teams. Having an innovative, proactive mindset is essential to this role.
