Anexciting opportunity has arisen for an enthusiastic and highly motivated individual to join our Cancer Services Team, within the Corporate Division of the Wirral University Teaching Hospital NHS Trust.
The successful applicant will act as Personalised Care Lead for the organisation providing leadership and driving the implementation and embedding of Personalised Care interventions and reduce variation across the organisation. This will include supporting tumour specific groups to implement all elements of the Personalised Care and support planning including Holistic Needs Assessment, End of Treatment summaries and Health and wellbeing clinics to support risk stratified pathways. The post holder will scope the current position in terms of Personalised Care interventions and build upon current practice to ensure the delivery of quality cancer services at WUTH, working with system partners and involving patients to co-design an Integrated Personalised Care offer for all Wirral patients affected by cancer.
A background in oncology and project management would be an advantage. Excellent communication skills and an ability to prioritise workload whilst working as part of a team are essential. It is also essential to have basic IT skills.

The post holder will act as Personalised Care Lead for the organisation, providing leadership and embedding change with all elements of Personalised Care and support planning. They will map and analyse the current processes and pathways in terms of Personalised Care interventions and build upon current practice to ensure the delivery of quality cancer services at WUTH as well as working in partnership with Macmillan Cancer Support Charity and the Community Integrated Personalised Care team to develop a shared vision of Integrated Personalised Care with the Wirral Community, informed by people with cancer.
The successful applicant will work with Macmillan Cancer Support Charity to ensure that the interventions meet the requirements of the national evaluation. They will also work with the IT Project Manager to explore digital solutions and implement and support more joined up care, including a remote surveillance system, ensuring data is captured, recorded accurately, monitored and shared across the system.
The post holder will be required to ensure the active engagement and involvement of people and service users via patients forums or focus groups and multidisciplinary team members, to identify continuous sustained quality improvements and solutions. They will also develop education and training for staff undertaking Personalised Care and support planning and share learning and best practice across the Trust and nationally.
