Job description


The post holder is responsible for the delivery of nursing practice within the socially vulnerable communities and ensuring that everyone is treated with dignity and needs are met in a timely manner. The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals of the social determinants within the vulnerable groups.
The practitioner will work collaboratively and maintain liaison and co-operation with other statutory, voluntary and contracting agencies to ensure optimum level of care provided with other partners whose service delivery has a direct impact on Asylum Seekers and Vulnerable groups.
Support the Senior Nurse in delivering and supporting effective care to patients.
Assist with managing the healthcare service across the socially vulnerable groups that we support.
To deputise for nursing colleagues during any periods of absence.
To represent the primary and community care viewpoint at operational and strategic meetings when necessary.
Work closely with the wider multidisciplinary team to ensure safe effective delivery of health care, including mental health and public health services.
To act as an accountable role model and clinical expert for healthcare
Responsibility for the promotion and use of the audit cycle within clinical practice.
Support the development and implementation of Healthcare Policies and procedures.
Responsibility of offering the initial assessment/completion of the “All Wales Blue book” to Asylum seekers living in Newport.
Responsibility of providing cultural awareness training and needs of the socially vulnerable groups to other disciplines for example g.p practices staff, mental health teams and Accident and Emergency staff.
Work in conjunction with other disciplines in the assessment, planning, implementation and evaluation of healthcare service provision to ensure a smooth patient pathway through the services.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

• Act as advocate for patients, Carers’ and staff.
• To provide professional, clinical and operational leadership and support to members of the healthcare team.
• To support and advise any visiting professionals on healthcare issues including risk management and health and safety.
• To attend Primary and Community health care team meetings /forums as required and provide feedback to the rest of the team, outlining actions that may be implemented as a result of feedback.
• To provide a comprehensive Nurse Screening Service to Health Inclusion Service, undertaking a physical and mental health screening assessment carried out using a professional interpreting service as required.
• To ensure that all newly arrived Asylum Seekers and Refugees are registered with a GP practice and to facilitate vulnerable groups to register with a GP.
• To provide information, advice, education and support to primary and community care teams to enable them to provide health care which is sensitive to this population group, encouraging and supporting the use of professional interpreting services.
• To be aware of and demonstrate practice based advances in nursing.
• To identify health needs of the Practice population: planning, providing and evaluating the service in response to those needs.
• To provide leadership and support the healthcare team with the delivery of long term conditions management.
• To maintain and promote accurate record keeping and documentation following national and professional standards and local protocols and policies.
• To collect and input clinical data to clinical systems: to access and utilise all available clinical information to support care planning and patient management, and support others to do the same.
• To ensure effective and safe medicines management processes are adhered to.
• Manage patient flow through the department to ensure a safe working environment for staff and appropriate access for patients.
• To plan and provide education for patients and carers, to enable them to optimise health and prevent disease thereby promoting public health, and support other staff to do the same.
• To identify target groups within the socially vulnerable groups for vaccination e.g. flu vaccine, covid vaccine and children immunisations.
• To liaise with Public Health Wales, in the event of notifiable disease identification, to implement actions as recommended by Public Health Wales. To work with other staff to ensure reduction of risk to other patients and staff.
• To ensure all accidents and incidents are reported and to assist in the investigation and implementation of action plans as a result.
• To identify, establish and maintain registers of those patients with chronic disease, such as asthma, chronic obstructive pulmonary disease, diabetes, epilepsy and coronary heart disease.
• Responsibility to organise and run clinic sessions.
• To provide mentorship support to students while on placement
• To provide an outreach service to those with complex needs who finds it difficult to access services jointly with the multi-disciplinary team.
