Senior Practitioner - Hospital Discharge - £35 Per Hour

• ASYE Completed
• Extensive Post Qualifying Social Worker experience
• Adults Safeguarding, Hospital Teams, Complex Care
• Social Work England registered

NonStop's client is a hospital based in North West London that is looking for experienced Senior Social Workers for their Hospital discharge team, to facilitate safe and timely discharge of residents from all acute hospitals and works closely with other members of the multi-disciplinary teams from major hospitals.

We are looking for an experienced Social Worker with excellent communication skills, experience in carrying out Care Act assessments and creative support planning within the context of facilitating Hospital Discharges.

This role will offer you the opportunity to develop working within a multi-disciplinary setting. You will have supportive managers and you will work within a dedicated team of colleagues with good knowledge of the Hospital Discharge process.
