To support individuals with Learning Disabilities and their caregivers in achieving personal outcomes, enhancing independence, and developing coping skills. Ensuring compliance with legal and procedural frameworks, the Social Worker assesses needs, identifies interventions, and collaborates with stakeholders to deliver effective support.
Duties and Responsibilities:
Assess needs, risks, and options for individuals and families.
Assist in enhancing independence and coping skills.
Work closely with carers to identify and address support needs.
Participate in decision-making forums and team meetings.
Maintain accurate client records and comply with policies.
Support equality of opportunity and ensure health and safety standards are met.
