About the role:
This is an exciting opportunity to join the Older People Social Work Team in the Welshpool area. As part of the role you will be required to assess and care manage the complex needs of Older People and their carers to ensure their health, wellbeing and social inclusion alongside close partnership working..
About you:
- You will need to be a good team player and establish and maintain good working relationships.
- Have good communication skills with the elderly population and their family and or carers.
- Be able to work autonomously in managing a caseload.
- Be able to respond to urgent requirements of the job role in a timely manner.
- Be able to utilise professional assertiveness to justify decisions and uphold social work practice.
What you will do:
- You will work with Older People and carers to complete integrated assessments utilising strengths based, outcome focused practice.
- Undertake Mental Capacity Act assessments and Best Interest Decisions alongside advocates.
- Seek to enable the older person’s independence whilst adopting a creative and holistic approach.
- Undertake safeguarding investigations as required.
