JOB DESCRIPTION

Permanent contract | £47,320 to £49,270 per annum (Scale L) | 37 hours per week | County Hall, Norwich

Exciting Opportunity: Supported Living and Residential Review Team Manager.

 Are you ready for a fulfilling leadership role? An incredible opportunity has opened to join our Learning Disability Service as a Team Manager overseeing a dynamic Countywide reviewing team of social workers and assistant practitioners. 

About the Role:

  • As the Team Manager, you will be at the forefront of ensuring the well-being and best outcome for our customers within LD Supported Living and Residential Homes. Your responsibilities will include:
  •  Leading the team in conducting annual reviews for customers in Supported Living, Residential Homes, and Out of County Residential placements, ensuring compliance with Care Act requirements.
  •  Support staff to adhere to ADASS guidance regarding the review of Out of Area placements.
  •  Taking the lead on home closures, ensuring timely identification of suitable placements for individuals.
  •  Monitoring and managing daily issues for customers in accommodation-based services, actively promoting independence and facilitating moves to alternative settings such as Housing with Care.
  •  Keeping S117 reviews up-to-date for individuals in accommodation-based settings, ensuring shared funding as per protocol.
  •  Liaising with commissioners, working on both Supported Living and Residential provisions to monitor contractual matters and contribute to the development of new schemes
  • Collaborating with procurement for both Supported Living and Residential provisions, addressing and resolving issues to ensure quality provision.
  •  Providing support with void management in Supported Living, collaborating with provider management for both Residential and Supported Living, ensuring a seamless flow of communication.
  •  Being jointly responsible for the supervision of a dedicated team of 13 staff, working closely with our Practice Consultant. Managing recruitment processes, HR matters, mileage, holiday requests, PDPs using myOracle.
  •  Ensuring accurate data capture and providing comprehensive reports on completed reviews, overdue reviews, cost savings, and the number of individuals transitioning to more independent settings.
  •  Effectively managing and addressing complaints from providers and customers in a timely manner.
  •  Ensuring the highest standards of assessments and care plans, supporting the service manager and Head of Service, in demonstrating robust financial management related to the cost of care packages.
