Job description
To ensure that all actions are captured and recorded
accurately from virtual board rounds.
To produce high quality daily reports and information to
reflect NCTR/Discharge Ready and MDT NCTR numbers to share at
operational site meetings and knowledge management to
allow system reporting and support the delivery of the
service.
Ensuring effective patient flow and timely assessment for
patients waiting for discharge to assess (Pathway 3 beds)
utilising patient activity list.
To support the TOCH System Manager and Service Manager
with day to day tasks and projects within the transfer of
care hub.
1. To maintain nervecentre for transfer of care hubs, collating updates, actions and
comments from the Multi-disciplinary team (MDT)
2. Complete high quality and accurate daily reports for NCTR and MDT NCTR for Service
Manager/GM to share at operational site meetings and external stakeholders as required
3. To maintain and co-ordinate transfer of care hub including Medically fit for discharge
6. To ensure that all patients’ referrals are managed in line with Trust recording keeping
and information governance policies.
7. Improving and maintaining good customer service relations with the service users and
commissioners. Ensuring all service users are communicated to and well informed of the
service and expected waiting times, and that their referral is managed in a timely manner.
8. To monitor and ensure compliance with all key performance indicators for the service.
Compiling reports on mandatory training adherence, clinical supervision adherence, site
inspections and recovery plans for use by service manager.
10. To monitor and risk assess all sites so they are fit for purpose implementing action
plans as required to ensure this. Including liaison with estates, league of friends and
alternate agencies to furnish sites.
