SalaryI’m currently looking to recruit a band 4 Health care assistant / Dementia navigator to join the dementia service in Hackney. The role is Monday – Friday, 09:00am – 17:00pm, 37.5 hours per week.
Job Summary: Maintain and effectively manage a caseload, providing an action plan to people affected by dementia that enable them to adapt to changes they may experience and provide appropriate support based on their changing needs.
To provide structured on-going support and follow up reviews tailored to the needs of person affected by dementia as required.
